For good and valuable consideration I acknowledge as received, and by signing this release I grant as follows:
1. Grant of rights
I irrevocably grant to the Photographer and the Photographer's licensees, successors and assigns the right to license and use still photographs, motion photographs or film, and my name, image and likeness, in any and all media and for any lawful purpose, including limitation, art, stock, advertising, trade and promotion, in perpetuity. My personal use is limited to prints, negatives and digital files delivered to me; I may not authorize others to use them. All rights, including copyright, are owned by the Photographer.
2. Editing and publication
No advertisement, product or other material using the images requires my further approval. I release the Photographer from liability for distortion, illusion or adverse results caused by publication, distribution or broadcast. The images may be combined, cropped, altered or modified. I consent to use of my name or a fictitious name.
3. Personal information
My personal information will not be made publicly available, but may be used as required or necessary to fulfill the purpose of this release, including sharing it with licensees and assignees and transferring it to countries with different data protection or privacy laws.
4. Release of liability
I release, discharge and hold harmless the Photographer, licensees, sublicensees, successors and assigns from all liability connected with use of the images, including claims for defamation or invasion of privacy. The released parties are not responsible for unauthorized use or piracy.
5. Warranties and binding effect
I warrant that I am of full legal age and have the right to enter into this agreement. I have read this authorization, release and agreement and am fully familiar with its contents. This agreement is binding on my heirs, successors and assigns.
6. Independent contractor and safety
I am an independent contractor and am not eligible for insurance or benefits from the Photographer. I am responsible for my own taxes, insurance, benefits and costs. I release the Photographer from liability for physical injury arising from the shoot.
